This will ensure that:

• all input elements take up the same space (e.g. login name, MFA code, password, ...)
  => this also applies to the "lost password" and "register" views
• all labels are positioned the same way (same padding, margins and no different floats)
• BS2 specific: make the help labels also have a lighter color. That way actual labels and help indicators are clearly separate.
• all elements are aligned identical, and use the Bootstrap grid correctly.
